The placental blueprint

Fetal growth restriction (FGR) is a pregnancy complication where the fetus does not grow properly due to placental insufficiency, meaning the placenta cannot supply enough nutrients and oxygen. Fetuses with FGR are at an increased risk of complications, including hypoxia and stillbirth. However, not all small fetuses are affected by placental insufficiency; some are constitutionally small but healthy. Distinguishing between these two groups is crucial as only the first group benefits from extra monitoring and interventions.

Doppler ultrasound is an essential tool for making this distinction. It measures the direction and speed of blood flow in the umbilical cord and fetal brain vessels. The research of Mauritia Marijnen demonstrates that combining estimated fetal weight with Doppler measurements is an effective method for identifying FGR due to placental insufficiency. A predictive model that includes estimated fetal weight, maternal biomarkers, and the most abnormal Doppler measurement offers the most accurate prediction of perinatal complications. Such a model may help clinicians make targeted decisions regarding the timing of birth. For instance, if Doppler measurements are normal, birth can safely be delayed until 40 weeks, if all other checks remain normal. This allows the baby more time to mature and increases the chance of spontaneous labor.

Furthermore, the research highlights the importance of structured placental examination after complicated pregnancies. This can offer valuable insights for managing future pregnancies. We also found that certain high-grade placental abnormalities affect Doppler measurements, further emphasizing the value of Doppler ultrasound in distinguishing healthy small fetuses from those growth restriction.
