Intensive insulin therapy and glucose management. Studies with the inplantable pump and a glucose sensor
In this thesis different aspects of continuous intraperitoneal insulin infusion (CIPII) as an option for intensive insulin therapy in Type 1 diabetes mellitus were investigated. Furthermore, the effectiveness and accuracy of (real time) continuous glucose monitoring (CGM) in both patients with T1DM using CIPII and a population undergoing cardiac surgery where studied.
We found improvement of glycaemic control with CIPII. Furthermore, quality of life and treatment satisfaction were high. Costs of CIPII were much higher than with continuous subcutaneous insulin infusion. CGM did not improve glucose regulation in patients with CIPII, although patients had high treatment satisfaction and would recommend CGM use to others.
In conclusion, continuous intraperitoneal insulin infusion is a good alternative for patients that do not reach good glycaemic control with other intensive insulin regimens. We weren’t able to detect improvement of glucose regulation with CGM in patients with CIPII or undergoing cardiothoracic surgery.
