Lung health in rural Nepal. Multi-state modeling of health status and economic evaluation of integrated respiratory care guidelines

PhD ceremony: Samir Kumar KC, Academiegebouw, Broerstraat 5, Groningen

Thesis: Lung health in rural Nepal. Multi-state modeling of health status and economic evaluation of integrated respiratory care guidelines

Promotor(s): prof. F. Willekens, prof. L. Niessen

Faculty: Spatial Sciences

Samir Kumar KC presents the findings of a cost-effectiveness analysis of an integrated lung health strategy, i.e. the Practical Approach to Lung health (PAL) strategy, developed by the World Health Organization (WHO) and implemented in a rural pilot setting in Nepal. Lung diseases, pneumonia, tuberculosis, chronic obstructive pulmonary disease (COPD), and asthma pose major health problems in Nepal, especially in rural areas where proper facilities and well-trained health staff are lacking. The book presents the results of the economic evaluation of the PAL pilot in terms of costs, health effects, and cost-effectiveness as compared to existing care.

Considering the results of this PAL pilot and the conditions for a full-scale implementation of PAL – especially the limited dedication and potential commitment of the national and international implementers – the PAL strategy deserves a benefit of doubt. The partially positive results and the possibility for further improvement at different levels of implementation should help keep the next PAL implementation phase flexible, taking into account the lessons learnt and allowing for future evaluation and monitoring.
