official university transcript which includes a list of all courses taken, the credit hours, the final grade received, an explanation of the grading system used, and an indication of your class rank

if English is not your native language, proof of proficiency must be included (TOEFL test or equivalent); Dutch students are exempt from this requirement

make arrangements to have three letters of recommendation sent directly to the Ms. D.F. Jansen; it is the responsibility of the student to ensure that the letters of recommendation arrive well before the application deadline.
